1. Health History and Authorization Form: To be completed and signed by the student and/or parent if the student is under 18 (Does NOT require a physician's signature.) Upload the document in the Student Health Portal.

2. Proof of a Physical Examination:  Proof of a general physical exam, is required for all students who live in university-sponsored housing. The form must be completed within 12 months of your first semester living on campus. For example: If you move in August 2024, then your physical must be dated on or after August 1, 2024.  This form MUST be signed and stamped by your physician’s office. Upload the document in the Student Health Portal.

3. Immunization Records:

Proof of the following three records, signed and stamped by your physician. Upload the document in the Student Health Portal.

• Two MMR (measles, mumps, rubella) vaccines or proof of immunity via a blood test ("titers")
• Meningococcal ACWY vaccine is dated within five years of your first semester living on campus. For example: If you move in August 2024, then your current meningitis vaccine must be dated on or after August 1, 2019
• Hepatitis B Vaccine series (3 doses)

 Your record will not be considered compliant unless supporting documents are uploaded and verified by the Student Health staff.
